Case Manager - Egg Harbor Township
Acenda Integrated Health offers over 100 behavioral health programs across New Jersey, including mental health services, crisis care, substance use recovery, family therapy, maternal and pediatric support, and residential programs.
Program OverviewKeep families together by providing safe, permanent housing and ongoing evidence‑based trauma‑informed support services to child protection and permanency families throughout the region.
Position SummaryProvide case management and support services to families with a history of homelessness or housing instability. Assist parents in transitioning to self‑sufficient living and improving individual and family stability. Offer coaching and guidance on life skills such as home management, nutrition, budgeting, parenting, education/vocation training, and advocacy.
Co‑facilitate groups for parents, children, and families. Link families to formal and informal community resources. Collaborate with KFT peers, DCP&P, and community service providers. Complete documentation consistently with program and agency requirements, using evidence‑based practice.
Key Responsibilities- Serve as the primary contact point, advocate, and informational resource for clients, care teams, family/caregivers, and community resources regarding the client’s service plan.
- Partner with clients to monitor adherence to service plans, evaluate effectiveness, monitor progress in a timely manner, and facilitate changes as required.
- Facilitate client linkages, referrals, and access to appropriate treatment, community supports, and medical or specialty providers.
- Maintain an active caseload of approximately 10–12 families, providing in‑person home therapeutic sessions to address treatment plan goals.
- Educate patients and family/caregivers about relevant community resources.
- Cultivate and maintain relationships with external community providers and supports.
- Complete and submit accurate reports and chart documentation in a timely manner according to program guidelines and established procedures.
- Ensure quality and accuracy of written and verbal communications.
- Support the quality/compliance/risk function as required.
- Perform other related duties as assigned.
- Bachelor’s degree; practitioner with at least 2 years of experience in a social service‑related field is preferred.
- Valid driver’s license with an acceptable driving record.
- Independent thinking, good judgment, and the ability to analyze work, set goals, develop plans, and utilize time effectively and efficiently.
Base Pay: $19.23 per hour
Sign‑on Bonus: $500

Bilingual rate:
Additional $1.50 per hour if Spanish proficient.
