We are seeking a candidate who would be responsible for the overall management of a patient's case. Responsibilities include coordination of care with the client, family, and other providers.

• Coordinate care with providers, including:
  Registered Nurses (RN), Licensed Practical Nurses (LPN), and Licensed Practical Nurses (LPN/LVN)
• Maintain case documentation in accordance with state regulations.
• Develop and maintain care plans for each client on the basis of clinical assessment and family needs.
• Coordinate care with other providers as necessary.
• Maintain appropriate communication with the admitting physician or psychiatrist regarding progress reports, changes in status, and refer patients to other providers or services as necessary.
• Conduct home visits as necessary to ensure optimal outcomes.
• Review patient charts weekly to ensure appropriate follow‑up and referral as necessary.
• Ensure that appropriate follow‑up is provided by all other providers as necessary.
• Coordinate care with community agencies as necessary to ensure appropriate follow‑up.
• Provide training to staff on client management and referral processes.
• Develop, modify, and maintain case files as needed in accordance with state regulations.
• Adhere to all policies and procedures of the agency or organization that employs you.

• Registered Nurse: 2 years (Preferred)
• Licensed Practical Nurse: 2 years (Preferred)
• Case Managers: 2 years (Preferred)
