• Provide direct patient care to residents, including administering medications, wound care, and vital sign monitoring
• Develop and implement care plans in collaboration with the healthcare team
• Communicate effectively with patients, families, and healthcare providers to ensure comprehensive care
• Participate in Shift Reports and handovers to maintain continuity of care
• Collaborate with Nursing Assistants and other caregivers to ensure a safe and supportive environment
• Monitor and report any changes in patient condition or concerns to the Nursing Supervisor
• Maintain accurate and up-to-date patient records and reports

• Current LPN licensure in the State of Arkansas
• Minimum 1 year of experience in a long-term care setting
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
• Basic computer skills and experience with electronic medical records
