Part-Time Social Services Director

Courtyard Rehab & Health
Schedule: 3 days per week, 8:00 AM – 5:00 PM (24 hours/week)

Position Summary

Courtyard Rehab & Health is seeking a compassionate, organized, and motivated Part-Time Social Services Director to join our team. The Social Services Director is responsible for enhancing the quality of life for our residents by providing emotional support, discharge planning, advocacy, and assistance with psychosocial needs while working closely with residents, families, and the interdisciplinary care team.

Responsibilities
  • Complete psychosocial assessments and develop individualized care plans.
  • Assist residents and families with adjustment to the facility and changing health conditions.
  • Coordinate discharge planning and community resource referrals.
  • Advocate for residents' rights and ensure dignity and respect are maintained.
  • Participate in care plan meetings and interdisciplinary team meetings.
  • Provide support and counseling to residents and families as needed.
  • Assist with advance directives and resident/family education.
  • Maintain accurate and timely documentation in accordance with state and federal regulations.
  • Investigate and document grievances and assist with conflict resolution.
  • Ensure compliance with CMS, state, and facility regulations regarding social services.
  • Collaborate with admissions, nursing, therapy, activities, and administration to meet resident needs.
Qualifications
  • Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field preferred.
  • Previous experience in a skilled nursing facility or long-term care setting preferred.
  • Knowledge of CMS regulations and discharge planning is preferred.
  • Strong communication, organization, and interpersonal skills.
  • Ability to work independently while collaborating with an interdisciplinary team.
  • Compassionate, dependable, and resident-centered approach to care.
Schedule
  • Part-Time
  • Three days per week
  • 8:00 AM – 5:00 PM
  • Days may be flexible based on facility needs.
