Job OverviewThe Activities Coordinator is a member of the PACE Interdisciplinary team (IDT) and is responsible for actively participating in all care coordination activities, including assessments and care planning. This role designs, implements, and oversees a variety of engaging activities tailored to the physical, cognitive, social, spiritual, and overall well-being of PACE participants. The program includes arts and crafts, animals, sports, games, dance and movement, music, and community outings.
The coordinator will plan a schedule of social and other purposeful independent or group activities and promote an engaging environment. Remote technology may be used as appropriate. The coordinator will adhere to PACE program guidelines, regulatory requirements, and organizational policies and procedures under the supervision of the PACE Care Center Supervisor at PACE centers.
- Actively participate in care coordination activities within the IDT and contribute to assessments and care planning.
- Design, implement, and oversee activities addressing physical, cognitive, social, spiritual, and overall well-being.
- Utilize diverse activity methods (arts and crafts, animals, sports, games, dance/movement, music, community outings).
- Develop and maintain a planned schedule of social and other purposeful activities for individuals or groups.
- Promote an engaging environment and, when appropriate, use remote technology.
- Adhere to PACE program guidelines, regulatory requirements, and organizational policies and procedures.
- Operate under the guidance and supervision of the PACE Care Center Supervisor.
Minimum of a High School diploma or equivalent. A bachelor's degree is highly desirable.
Minimum of 1 year of experience in a social or recreational program providing and coordinating services for a frail or elderly population (preferred). Experience with dementia care is highly desirable.
Completion of 36 hours of Activity Leader (previously "Activity Director") course, designed specifically for this position and approved by the State of California, is required within 30 days of hire.
Bilingual English/Spanish/Mandarin/Cantonese, depending on location preferred.
Experience in providing virtual activities is preferred.
A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.
$25.75 - $32.19 hourly
