Partner doc is filling in for the vacancy, want new doctor

• Anticipated start date: ASAP

• Current staff:
o Doctors: 1 partner doc (will leave when new doc is hired)
o Support Staff: at least 1 hygienist (didn t specify)

Current Schedule M-F: 9-4

• Patients per day:

• Types for Procedures:
Bread & butter dentistry currently referring out braces, in house aligners

• Patient age range: all ages most adults

• Technologies: CT & Digital Scanners, iTero, Electric-Handpiece, Rotary Endo

• Clinic production: $700,000/year

• Payor make-up: (Private insurance, FFS, Medicaid, etc ): PPO & cash pay, Medicaid (about 30%)

Offering:

• Daily Guarantee/% of production or collection:
Will do either; $15-20k/month base, or 30% of production minus lab fees all doctors are 1099

• Earning Potential: clinic currently doing $700k/year

• Equity or Partnership:
