Personal Health Care Attendant (PHC)

Requirements- Prefer 1-2 years of personal care experience.
- High school diploma or equivalent preferred but not required.
- Spanish bilingual preferred.
- Must submit proof of driver's license and current auto insurance if means of transportation is an automobile.
- Must have reliable means of transportation to ensure timely arrival to client's home according to visit schedules.
- Must submit to a background check.
- Must submit to a drug test.
- Skillful in organization of work and in principles of time management.
- Ability to contribute to the quality of care through constructive communication with primary care clients.
- Good communication skills and ability to establish rapport with clients.
- Demonstrated ability to work with patients and employees, and to work in a proactively diverse and inclusive organization.
- Must practice a high level of confidentiality and be able to respond to questions from clients; must be able to work with others to resolve issues and maintain composure in a changing priorities environment.
- Must have knowledge of all company policies and procedures, work on a timetable, follow instructions, respond to management direction, and solicit feedback to improve performance.
- Excellent character references and solid work background.
- Must be able to read, write and speak English. Spanish is preferred.
- Must have knowledge and proficiency of office computer equipment and software.
- Minimum of eighteen (18) years of age or older.
- Must be reliable, punctual, neat, organized, willing to perform tasks as outlined in each client’s plan of care.
- Attendant must comply with state‑mandated electronic visit verification system (LAN LINE or TOKEN).
- Report any change in condition or problems with client to supervisor within 24 hours of awareness.
- Report any emergency/life‑endangering situation to 9‑1‑1 and then notify office immediately.
- Complete all duties listed in the individualized service plan for every client.
- Assist with activities of daily living: hygiene (bathing, shaving, brushing teeth, etc.), dressing, toileting and assistance with clothing during toileting.
- Transfer/transport clients safely using appropriate devices; aid with wheelchair, walking, and exercise.
- Clean client’s living area, kitchen, bathroom, and laundry; change linens and maintain a safe environment.
- Prepare food, cook, wash dishes, and remind clients to take medications.
- Run errands, purchase groceries, accompany clients to doctor or clinic.
- Maintain client confidentiality and follow HIPAA rules and company procedures for lifting and moving clients.
- Adhere to safety and infection‑control procedures, including hand hygiene, use of disposable gloves, and proper disposal of items.
- Perform other duties as assigned; duties may change at any time with or without notice.
This is a 24‑hour facility (Flexible).
