Physical Therapist (PT) – Supervisor

Angels of Care Pediatric Home Health is hiring a Physical Therapist (PT) – Supervisor to provide in‑home Physical Therapy services while supporting and developing a team of clinicians.

This role is ideal for an experienced Physical Therapist (PT) passionate about delivering high‑quality care while mentoring, guiding, and supporting therapy staff in a collaborative, patient‑centered environment.

What You’ll Do Clinical Responsibilities
  • Provide evidence‑based Physical Therapy services in the home and community
  • Perform evaluations, reassessments, and develop individualized treatment plans
  • Deliver interventions to improve mobility, strength, balance, coordination, and functional independence
  • Support pediatric patients in developing or regaining physical and neurological function
  • Educate and empower families and caregivers for carryover
  • Coordinate care with physicians and referral partners
  • Develop and instruct home exercise programs
  • Document all care and services in an electronic medical record system
Leadership & Supervision Responsibilities
  • Provide clinical oversight and mentorship to therapy staff
  • Support onboarding, training, and development of clinicians
  • Review documentation and ensure compliance with standards and regulations
  • Collaborate with clinical leadership to maintain quality of care
  • Assist with caseload management and team support needs
  • Participate in performance development, coaching, and quality improvement initiatives
Qualifications Required
  • Active, unrestricted Physical Therapist license (Texas)
  • Graduate of an accredited Physical Therapy program
  • Current CPR certification
  • At least 1 year of experience working as a Physical Therapist
Preferred
  • 1+ year of pediatric experience
  • Home health experience
  • Bilingual:
    English and Spanish
