In this role, you'll assist with daily living activities, implement care plans, and support behavioral and rehabilitative services under clinical guidance. You'll document progress, collaborate with interdisciplinary teams, and partner with families to promote independence and quality of life. Responsibilities

• Provide hands-on support with daily living activities in home and community settings
• Follow person-centered care plans under clinical or supervisory direction
• Support individuals with behavioral health and brain injury rehabilitation needs
• Accurately document services, progress notes, and incident reports
• Collaborate with supervisors, clinicians, and families to adjust supports
• Promote safety, dignity, and independence in all interactions
• Assist with transportation and community integration activities
• Maintain infection control and adhere to all health and safety protocols
• Participate in required training and ongoing professional development
