Wraparound is not a service or program; rather, it is a definable process that results in a unique set of community services and natural supports that are individualized for a child and family to achieve a positive set of outcomes. The Wraparound Facilitator will work with the provider community to develop responsive and flexible resources that facilitate community- based interventions and support which correspond with the needs of the youth and family.

Care coordination using a wraparound philosophy enables a timely response to the needs of the family and youth across several life domains, incorporates formal and informal supports, and develops personalized interventions and strategies that promote the youth succeeding in their community. This position is responsible for working in full partnership with team members to develop a plan of care, overseeing implementation of the plan, identifying providers of services or family-based resources, facilitating child and family team meetings, and monitoring all services authorized as per the youth's level of care.

The Wraparound Facilitator assures that services are delivered consistent with being strength-based, family driven, youth guided, and culturally competent. The Wraparound Facilitator is an advocate of wraparound values and principles of the model.

This class works under close to general supervision according to set procedures but determines how or when to complete tasks.

The functions listed below are those that represent the majority of the time spent working in this position. Management may assign additional functions related to the type of work of the position as necessary.

• Utilize the wraparound planning process to deliver intensive case management in accordance with applicable Texas Administrative Code standards and the Texas Resiliency and Recovery Management Guidelines. This includes taking steps that are necessary to assist the youth and family in gaining access to needed services and connecting with service providers, making referrals, arranging and facilitating linkage to community supports, and meeting weekly, more often if needed, with the youth and family.
• Maintain compliance with training set forth by HHSC, Medicaid, and the National Wraparound Implementation Center (NWIC). This includes successfully completing the initial introduction to wraparound training within the first three months of being hired as a wraparound facilitator and participating in ongoing training offered by entities listed.
• Meet standards of care and timelines set forth by HHSC, Medicaid, and the National Wraparound Implementation Center (NWIC). This includes but not limited to completing a clinical eligibility assessment within seven business days of a youth having a positive demographic eligibility screening, submitting revised individualized plans of care at minimum every 90 days or as needed determined by the team in the clinical management of behavioral health services system, update clinical eligibility assessment for reauthorization of YES services annually, develop an effective crisis and safety plan during initial meeting and update crisis and safety plan after each crisis event that the youth experiences.
• Submit 95% of all critical incident reports to HHSC within 72 hours or three business days of being notified of a critical incident.
• Schedule and conduct 95% of all emergency child and family team meetings within 72 hours but no later than 7 business days following a crisis event. The wraparound facilitator must be flexible, willing and able to conduct an emergency child and family team after hours if requested by the youth or family.
• Perform high-fidelity wraparound services based in a trauma-informed, strengths-based, and individualized care model.
• Assures that care is delivered consistent with strength-based, family-driven, youth-guided, and culturally competent values.
• Participate in monthly staffing's with community providers and maintain open, respectful communication with all team members as needed to provide and receive updates on the youth and family.
• Offer consultation and education to internal EHN staff and community partners regarding the wraparound's values and principles.
• Ability to effectively communicate both verbal and written thoughts, ideas, and facts. Writes and presents information concisely. Interprets and understands written information and can listen attentively to verbal and non-verbal cues that lead to a deeper understanding. Possess excellent training and group facilitation skills.
