Avir at Montana is seeking a dedicated and compassionate Per Diem / PRN Occupational Therapist (OT) to join our rehabilitation team. The Occupational Therapist will provide skilled evaluation and treatment services to patients with a variety of functional and rehabilitation needs, helping them achieve greater independence, improve daily functioning, and enhance their quality of life.
Essential Responsibilities- Evaluate patients' functional abilities and develop individualized occupational therapy treatment plans.
- Provide skilled occupational therapy interventions based on each patient's diagnosis, condition, and goals.
- Address activities of daily living, strength, coordination, balance, mobility, cognition, and functional performance.
- Monitor patient progress and adjust treatment plans as clinically appropriate.
- Complete accurate and timely evaluations, treatment notes, progress reports, and discharge documentation.
- Collaborate with physicians, nursing staff, physical therapists, speech-language pathologists, and other members of the interdisciplinary team.
- Educate patients, families, and caregivers regarding therapeutic exercises, adaptive techniques, safety strategies, and home programs.
- Participate in care planning and interdisciplinary meetings as needed.
- Maintain compliance with applicable federal and state regulations, facility policies, and professional standards.
- Provide coverage on a PRN/per-diem basis according to facility and patient needs.
- Current Occupational Therapist license in the State of Texas
. - Graduate of an accredited Occupational Therapy program.
- Current CPR certification, if required by facility policy.
- Previous experience in skilled nursing, rehabilitation, long-term care, or a related healthcare setting preferred.
- Strong clinical, communication, organizational, and documentation skills.
- Ability to work independently and effectively as part of an interdisciplinary team.
- Dependable, flexible, compassionate, and committed to providing high-quality patient-centered care.
