PRN Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN)

As an LPN/LVN, you will provide direct patient care under the supervision of a Registered Nurse, delivering treatments, assisting in assessments, and supporting interdisciplinary care planning in accordance with patient needs.

• Administer medications safely, accurately, and based on patient age and clinical condition.
• Minimize potential for medication errors by following proper medication administration procedures.
• Assist the RN with initial assessments and patient reassessments per policy.
• Identify and implement appropriate rehabilitation nursing interventions.
• Perform timely and accurate Quality Improvement (QI) assessments under RN supervision.
• Deliver care based on patient response to treatment and update the interdisciplinary plan accordingly.
• Assess and reassess pain; use appropriate pain management techniques and educate patients and families.
• Perform all patient care tasks in a manner that promotes patient safety and reduces medical errors.
• Develop and implement individualized teaching plans based on patient and family learning needs.

• Graduate of an accredited Licensed Practical/Vocational Nurse program.
• Minimum six months of rehabilitation experience; acute care setting preferred.
• Current state licensure as a Licensed Practical/Vocational Nurse.
• CPR/BLS certification required.
• IV Certification highly preferred or must be obtained within six months.
• Strong clinical skills and commitment to safe, patient‑centered care.
• Ability to work effectively in an interdisciplinary team environment.
