Job Description Plan, implement, coordinate, evaluate and promote the continuity of care consistent with established standards and practices. Assess health status by interviewing patients regarding current status and medical history. Assess patient/family needs and provide teaching and instructional material. Provide psychosocial support to patients and families. Manage patient medication refill requests from the pharmacy telephone line. Facilitate and schedule consultations, diagnostic tests, and referrals for patients.

Triage patient phone calls and document relevant information in the patient's chart. Critical Information On-site position. Education/Licenses Needed Graduate from an accredited program for professional nursing education, BSN preferred. Current RN state license with the State Board of Nursing in practicing state. Current Basic Life Support (BLS) certification. Benefits:
