Job Description
This position is available for Morning, Evening, and Night shifts on multiple homes. Nurses may earn additional pay for evenings, nights, weekends, and maximum security locations. The role serves people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems at 13 State Supported Living Centers across the state.
The Direct Care RN performs routine nursing work providing assessment, care, and treatment of individuals residing in a State Supported Living Center (SSLC). Responsibilities include evaluating, treating, documenting healthcare needs, and reporting to the appropriate person(s). The RN communicates with direct care staff, physicians/PCP, administration, and other nursing staff to ensure optimal care as part of the interdisciplinary team. The RN also notifies the supervisor and PCP of trends and concerns, completes required documentation, and maintains records.
Other duties involve assisting LVNs and Certified Medication Aides (CMA), participating in facility quality assurance/quality improvement (QA/QI) initiatives, providing treatment to HHS employees injured in the course of employment, responding to emergency situations to ensure safety and care of individuals and staff, and working under moderate supervision of the Nurse Manager or Nursing Operations Officer.
Essential Job Functions (EJFs)- Attend work on a regular basis; work a specific or rotating shift schedule, extended shift, and/or overtime as required.
- Serve in an on‑call status on a rotating schedule to meet operational needs.
- Perform routine nursing tasks and procedures per HHSC, SSLC, and state regulations (e.g., tracheostomy care, gastrostomy/jejunostomy care, wound care, breathing treatments).
- Collect specimens for laboratory examination and conduct appropriate screening tests.
- Administer medications, immunizations, and treatments; observe patients for unusual symptoms and reactions.
- Make rounds and/or home visits as needed; work in other SSLC locations as required.
- Monitor individuals’ weight, diet, feedings, and vital signs; use nursing judgement to provide adequate care and initiate acute care plans when necessary.
- Assist in evaluating nursing intervention outcomes and provide input to the healthcare team with recommendations for improvements.
- Instruct direct support professionals on monitoring individuals after treatment or procedure using instruction sheets.
- Document and report treatments and observations accurately in the electronic health record or paper chart.
- Assist LVNs and/or CMAs, and recommend improvements, including acclimating them to individuals’ needs and preferences.
- Assist LVNs with implementation of protocols, procedures, and programs to improve overall health and safety of staff and individuals.
- Support Nurse Manager with controlled drug counts, medication counts, emergency checklist logs, refrigerator temperature logs, and glucose controls.
- Assist the Infection Control nurse and Quality Assurance Nurse in implementing disease prevention measures, therapeutic nursing programs, and facility QA/QI initiatives.
