Primary Care Medical Partners is seeking a compassionate, motivated, and patient-centered Family Nurse Practitioner (FNP) to join our growing outpatient primary care practice.

We are looking for a clinician who enjoys building lasting relationships with patients and providing comprehensive, high-quality care in a collaborative outpatient environment.

Position Details

  • Position: Family Nurse Practitioner – Outpatient Primary Care
  • Schedule: Monday–Friday
  • Employment: Full-time preferred; part-time option available
  • Benefits: Benefits package available for eligible full-time employees
  • Bilingual English/Spanish:Strongly preferred

Responsibilities

  • Provide comprehensive primary care to patients across the lifespan.
  • Evaluate, diagnose, and treat acute and chronic medical conditions.
  • Manage conditions such as diabetes, hypertension, hyperlipidemia, obesity, and other common primary care diagnoses.
  • Perform preventive health and wellness examinations.
  • Order and interpret laboratory tests, imaging, and other diagnostic studies.
  • Prescribe medications and provide ongoing medication management.
  • Provide patient education regarding medications, nutrition, lifestyle modifications, and disease prevention.
  • Coordinate referrals and collaborate with specialists when appropriate.
  • Maintain accurate and timely electronic medical records.
  • Establish strong, trusting relationships with patients and their families.
  • Work collaboratively with physicians, nursing staff, medical assistants, and other members of the healthcare team.

Qualifications

  • Master’s or Doctoral degree in Nursing from an accredited program.
  • Current and unrestricted Texas APRN/FNP license.
  • National FNP certification.
  • Current DEA registration and prescriptive authority.
  • Current BLS certification.
  • Previous outpatient primary care experience preferred.
  • Excellent communication and clinical judgment.
  • Fluent English and Spanish strongly preferred.
