$19 per hour

Summary

The Case Manager assists with the implementation and development of the Transitional Living Program by developing housing opportunities, partnerships, and a wide range of supports for homeless youth. The scope of work is subject to change as needed to ensure the project’s success and to meet funders’ requirements. Likewise, complementary changes may be made to projects or individual goals and employee expectations based on agency/management strategic goals.

All Center for Children team members are expected to be collegial and collaborative, mutually supportive, and focused on excellent services to clients, fidelity to program models, and ethical provision of services.

Responsibilities
  • Become an expert on TLP program standards via resources on the Health and Human Services website and the Runaway and Homeless Youth Training and Technical Assistance Center (RHYTTAC). Takes the lead on compliance reviews
  • Participate in the Performance and Quality Improvement processes, ensuring services are being delivered with fidelity to model and contractual/licensing obligations
  • Adhere to accreditation standards (as developed), licensing standards, and other policies/regulations
  • Foster an environment of support and accountability that encourages leadership, personal growth, and the development of independent living skills
  • Conduct and coordinate intakes, assessments, treatment/exit planning processes, and case management services
  • Assist youth in applying/registering for further education and financial assistance, obtaining employment, securing stable housing, learning independent living skills, and developing other community connections
  • Assist youth in identifying a physician and scheduling appointments. Provide individual and group classes on basic health care.
  • Support youth’s ability to address barriers and achieve established goals; refer youth to outside service providers and coordinate services as necessary; advocate for youth’s best interest
  • Oversee progress through regular contact and meetings with youth; maintain files and supportive documentation chronicling progress
  • Develop and implement programs that benefit youth’s transition to self-sufficiency
  • Coordinate life skills and social skills groups
  • Provide individual and/or group counseling and parent/child counseling
  • Provide aftercare follow-up contact and services
  • Ensure accurate tracking and documentation of all referrals and services provided
  • Ensure case records, files, and data entry programs are maintained properly and in accordance with the confidentiality policy and that all statistical information and data required by any government agency or funding source is completed and submitted on time
  • Work collaboratively with on-site programs to understand the needs of youth and assist in providing placements when needed
  • Coaches and trains staff on behavior management techniques, crisis de-escalation, youth engagement, TLC operations, documentation, and other skills needed to work effectively in the TLP
  • Participate in outreach, community education activities, and key organization events
  • Maintains a high level of personal accountability, seeks opportunities to learn, welcomes opportunities to improve/correct performance/behavior, and demonstrates thoughtfulness in making suggestions to improve the quality of services
  • Maintains extremely high standards and levels of attention to detail, creating a welcoming and healthy youth development environment in the TLP
  • Utilizes Motivational Interviewing, the Seeking Safety curriculum, positive youth development, trauma-informed care, and key behavior management strategies and techniques for de-escalating crises.
  • Participate in multidisciplinary team meetings, working closely and cooperatively with other stakeholders to ensure we are a viable and valuable partner in preventing and ending homelessness and advocating for crime victims
  • Remain calm in a crisis, offering support and reassurance to clients and staff during periods of high stress
  • Manage, with the support of the Project Manager, planned and unplanned site visits from contract monitors.
