POSITION OVERVIEWThe Diversion Hub is a comprehensive, one-stop network dedicated to assisting justice-involved individuals in Oklahoma, Canadian, and Cleveland Counties by harnessing the power of multiple support agencies through combined and synchronized services. Our goal is to provide life-stabilizing resources while empowering individuals to reduce their encounters with the criminal legal system through enhanced support services, including case management, data integration and coordinated communication under one roof.
Case managers will provide direct case management services to clients and act as the primary point of contact and liaison between the client, Justice Navigator, and the service navigators.
- Demonstrate the values expected within the program and promote the mission of the organization.
- Conduct regular intakes of clients, document progress notes, develop service plans, report critical incidents, collaborate with partner service navigators, and maintain complete and comprehensive client records.
- Conduct client assessments to aid in the creation of individualized service plans focused on client safety, stability, and self-sufficiency, while assisting the client in getting out of the criminal legal system.
- Evaluate client risk and assess needs for immediate intervention.
- Attend court with clients and advocate on their behalf.
- Prepare necessary documentation to provide to the court on behalf of a client, collaborating closely with the Justice Navigator and any onsite service navigators.
- Serve as an ongoing liaison to partner agencies, attorneys, law enforcement, and all referring sources.
- Maintain adequate communication channels with Justice Navigators, onsite service navigators, clients, and all referring agencies.
- Submit all required case management reports timely.
- Facilitate client access to needed supports and resources such as housing, employment, government benefits, food, , clothing, referrals to mental health providers, referrals to physical health facilities, referrals to substance abuse services, vocational opportunities, drug tests, and court order classes.
- Assist clients in developing natural resources and navigating community social support networks.
- Always adhere to agency code of ethics and professional standards, including maintaining strict client confidentiality.
- Attend all staff development, trainings, and meetings as required.
- Perform any other duties within the scope, spirit, and purpose of the job and/or organization.
Education and Experience
- 1 to 3+ years of experience in social work, communication, criminal justice, political science, or a similar human service area.
Additional Job Requirements
- Clearance of background check.
- Must have own transportation for site visits. Valid driver’s license and insurance on a motor vehicle in accordance with state law and agency requirements.

- Intermediate computer, mobile device and internet skills with knowledge of software programs including but not limited to Windows and Microsoft Office Products, Cloud-based File Sharing such as Google Drive and SharePoint, Adobe Products, Client Databases.
While performing the duties of this job, the employee must be able to sit over long periods of time and be able to talk, hear, sit, stand, walk; use hands to finger, handle or feel objects, tools, or controls; reach with hands and arms; and stoop, kneel, crouch, or crawl; climb stairs, lift and or move items of up to 25 pounds.
Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus.
