Job Title
Conduct comprehensive participant intakes, assessments, and housing needs evaluations and develop individualized Housing Case Plans based on each family's circumstances, barriers, and goals.
Provide ongoing, client-centered case management services designed to promote housing stability, wellness, independence, and self-sufficiency.
Connect participants with appropriate community resources and supportive services, including mental health services, substance use treatment, domestic violence resources, childcare, employment assistance, benefits, and other social services.
Make referrals to community-based programs and follow up with participants and service providers to ensure successful connection and completion of services.
Track participant progress toward established housing and self-sufficiency goals and modify case plans as appropriate.
Complete participant recertifications every three months to evaluate continued eligibility, progress, housing stability, and ongoing need for assistance.
Educate participants regarding the program's rental assistance step-down model, including the gradual reduction of rental subsidies and expectations for increasing financial independence.
Assist participants throughout all stages of the permanent housing process, including identifying available housing, completing applications, gathering required documentation, communicating with landlords, advocating on behalf of participants, and coordinating leasing and move-in activities.
Work collaboratively with Housing Locators/Matchers to identify appropriate housing opportunities and overcome barriers to placement.
Develop and maintain relationships with property owners, landlords, and property managers to expand available housing opportunities for program participants.
Educate landlords and property managers regarding UBH rental assistance and placement programs, including short-, medium-, and long-term subsidy structures, program timelines, payment processes, and landlord participation requirements.
Communicate and negotiate with landlords regarding rental terms, participant eligibility, leasing requirements, and other housing-related matters.
Perform rent calculations for rental assistance programs and counsel participants regarding rent obligations, security deposits, lease provisions, and other housing-related financial responsibilities.
