Social Services Assistant is an essential role in our facility, providing compassionate and dedicated support to our patients and residents.
As a Social Services Assistant at Elba Rehabilitation and Wellness Center, you will have the opportunity to make a meaningful difference in the lives of our patients and residents. In this full-time role, you will provide emotional support, connect patients and residents with community resources, and facilitate their transition back to their homes. Your exceptional communication and interpersonal skills will enable you to build trust and rapport with our patients and their families, ensuring they receive the highest level of care and support.
Key Responsibilities:- Transport residents to appointments
- Provide emotional support and counseling to patients and residents, addressing their social and emotional needs
- Conduct assessments to identify patients' and residents' needs and develop personalized care plans
- Connect patients and residents with community resources, such as government assistance programs, support groups, and transportation services
- Facilitate patients' and residents' transition back to their homes, ensuring they receive necessary support and follow-up care
- Maintain accurate and confidential records of patient and resident interactions and care plans
- Collaborate with the interdisciplinary team to ensure patients' and residents' needs are met and care plans are implemented
- High school diploma or equivalent required; degree in social work, counseling, or a related field preferred
- 1 year of experience in social services, healthcare, or a related field
