Position: NURSE PRACTITIONER

Acute Access & Triage Collaborator – Part‑Time/Flexible Schedule | Outpatient Only – Eldersburg, Maryland

BW Primary Care, LLC is a physician‑managed primary care practice and subsidiary of Life Bridge Health. BW Primary Care is one of the largest and most established internal medicine and family medicine practices serving Carroll and Baltimore Counties. Position Overview

We are seeking an experienced Nurse Practitioner or Physician Assistant to join our team in a part‑time Acute Access & Triage Collaborator role. This position is designed to enhance same‑day access, support clinical triage, and improve care flow across our multidisciplinary clinician team. The role focuses on urgent and acute visits, post‑hospital follow‑ups, and inbox‑based clinical triage, helping ensure patients receive timely, appropriate care while supporting continuity with their primary clinician.

One or two part‑time clinicians may fill this position to provide coverage across the week.

Clinical Responsibilities
  • Same‑day urgent and sick visits (in‑person and telehealth)
  • Acute management of chronic disease exacerbations
  • Post‑hospital and post‑emergency department follow‑ups
  • Clinical triage and inbox support (messages, results, care escalation)
  • Coordination with physicians and advanced practice colleagues to expedite care
  • Care for adult patients (age 18+)
Position Highlights

Qualifications
  • Nurse Practitioner or Physician Assistant from an accredited program (required)
  • Active Maryland medical license (required)
  • CDS and DEA licenses (required)
  • Minimum 2 years of primary care or urgent care clinical experience
  • Excellent communication and teamwork skills
  • Comfort with EMR systems, inbox management, and technology
  • Ability to work independently while collaborating closely with a large clinical team
