Position: Float Physical Therapist (PT) - Outpatient

Major Responsibilities

  • Interprets physician referrals, as appropriate, and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations using appropriate methods.
  • Establishes a plan of care and implements treatment based on evaluation results and functional goals; monitors outcomes and adjusts treatment plans as needed; provides status updates to physicians and teammates.
  • Identifies factors affecting patient's physical and psychosocial comfort and attends to these needs.
  • Plans for and completes patient discharge including written home programs, patient/family teaching, referral to community services, transition planning within continuum of care, and ordering equipment as indicated.
  • Communicates and collaborates with stakeholders to achieve optimal patient care.
  • Completes all required documentation following department policy and accreditation standards – patient evaluations, progress notes, team conference reports, discharge summaries.
  • Records patient care and progress; completes daily statistics, billing, and scheduling; applies insurance guidelines regarding authorization and number of visits; maintains productivity standards.
  • Maintains clinic cleanliness and safety, performs manual techniques, patient positioning, transferring, and transportation; demonstrates skill in modalities, exercise equipment; follows safety and infection control procedures.
  • Participates in performance improvement activities to improve quality of care and manage caseload; informs leadership of coverage needs.
  • Pursues professional growth through continuing education, department in‑services, and professional literature; orients new staff and participates in coworker evaluations.
  • May supervise physical therapy assistants, extenders, and students, delegating patients and providing direction while maintaining responsibility for patient care plans.
Minimum

Job Requirements Education

Doctorate degree in Physical Therapy from an accredited school or educational requirements commensurate with date of graduation. Maintains continuing education per state professional standards.

Certification / Registration / License

Physical Therapy License in the State of Illinois or State of Wisconsin; must be licensed in state of work. Basic Life Support (BLS) for Healthcare Providers (obtained within 90 days unless department leader waives).

Knowledge / Skills / Abilities
  • Intermediate computer skills.
  • Excellent communication, interpersonal, and organization skills.
  • Demonstrated organization, decision‑making, and problem‑solving skills.
  • Successfully completes all competency assessments/requirements pertaining to provision of patient care and consistent with site standards.
  • Annual educational requirements for hospital‑based certifications (e.g., DNV, CARF).
  • Knowledge and skills to provide care appropriate to patient age; principles of growth and development across lifespan.
Physical Requirements And Working Conditions
  • Must be able to sit, stand, walk, lift, and squat throughout the workday.
  • Lift up to 50 lbs. from floor to waist.
  • Lift up to 10 lbs. from waist to overhead.
  • Lift and carry up to 40 lbs. at waist height over a reasonable distance.
  • Push/pull with 30 lbs. force.
  • Perform a sliding transfer of 150 lbs. with a second person present.
  • Repetitive use of hands: simple grasping (5–15 lbs., 20% of workday); pushing/pulling (up to 75–100 lbs., 10% of day); fine manipulation during massage and joint mobilization.
  • Functional use of senses for effective communication.
  • Exposure to electrical, chemical hazards, blood and body fluids; must wear protective clothing as needed.
  • May require travel; exposure to road and weather hazards.
  • Operates all equipment necessary to perform job.
