Physical Therapist – Evanston, IL

FOX Rehabilitation is growing in Evanston, Illinois and is seeking licensed Physical Therapists to provide individualized care to older adults in private residences and senior living communities through our Geriatric House Calls™ model.

• Deliver one‑on‑one physical therapy services to older adults in private residences, senior living communities, independent living, assisted living, and memory care facilities.
• Provide therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document patient care accurately within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and fitness specialists.
• Offer telehealth or virtual care services when appropriate to extend access and flexibility.
• Meet the physical demands of in‑home and community‑based care, including moderate to maximum assistance with patient transfers, bed mobility, and driving to patient homes.
• Maintain professional responsibilities and adhere to the Geriatric House Calls™ model requirements.

• Valid Physical Therapy license in the state of practice, or eligibility to obtain one.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Ability to meet the physical demands of in‑home and community‑based care.
• Strong interpersonal skills to build lasting patient relationships.
• Completion of FOX’s nationally recognized Emerging Professionals Mentor Program is a plus for new graduates.

• Competitive salary:
  Full‑time roles range from $65,000 to $105,000 annually, with potential for additional earnings based on performance.
• Full‑time, part‑time, and PRN (Fox Flex) options with salaried compensation for full‑ and part‑time and pay‑per‑unit for PRN.
