Registered Dietitian (RD) – Chronic Disease & Weight Management Programs

Location: Hybrid – Outpatient & Skilled Nursing Facility (SNF) settings | Telehealth available

Position Type: Full-Time

About the Role:
We’re seeking a compassionate and skilled Registered Dietitian (RD) to join our interdisciplinary primary care team. In this role, you’ll provide Medical Nutrition Therapy (MNT) and Behavioral Counseling for Obesity to patients managing diabetes, prediabetes, obesity, and other chronic conditions. You’ll deliver individualized nutrition care, support structured weight management programs, and help patients achieve sustainable lifestyle changes through evidence-based practice.

Our RDs serve both outpatients and patients residing in Skilled Nursing Facilities (SNFs), offering services in person and via telehealth to ensure continuity and accessibility of care.

What You’ll Do:

  • Conduct comprehensive nutrition assessments and develop personalized care plans.
  • Deliver MNT for patients with diabetes, renal disease, and other chronic conditions.
  • Provide structured behavioral counseling for obesity (G0447) following CMS guidelines.
  • Collaborate closely with physicians, nurses, and behavioral health professionals.
  • Document all care in EMR systems per Medicare and payer standards.
  • Support additional nutrition and chronic disease management programs.

Qualifications:

  • Bachelor’s or Master’s in Dietetics, Nutrition, or related field.
  • Registered Dietitian (RD/RDN) credential (CDR).
  • Active state licensure or certification.
  • 1–2 years of clinical experience (outpatient, primary care, or SNF preferred).
  • Strong communication and motivational interviewing skills.

Preferred: CDCES certification or experience with value-based care and chronic care management.
