Advocate Health is seeking a Home Health Aide to join the Hospice team in Illinois. The schedule is Monday–Friday 8a–4:30p with weekend and holiday on-call rotation, and a $2,500 sign-on bonus for eligible external candidates.
Responsibilities include assisting with daily living activities, monitoring patient status, and supporting the care plan under the supervision of an RN. Travel to patient homes is required; the department is Hospice Lake Zurich.
