Job Description

The Physical Therapist will treat patients using the principles and practices of physical therapy and in accordance with the patient’s age, developmental level, and diagnosis. Collaborates with the interdisciplinary team. License-eligible therapists would perform duties under the supervision of a licensed therapist in accordance with the Illinois Physical Therapy Practice Act.

This is a registry position - Hours 8:00 am - 4:30 PM variable days requiring 2-3 shifts, on weekends, per month.

Essential Duties and Responsibilities

1. Evaluates patients, establishes goals, and develops individualized treatment plans as prescribed by a licensed physician/practitioner.

2. Provides direct patient treatment based on current physical therapy practice and revises treatment plan based on the patient's response.

3. Provides education to patient and family/caregiver regarding patient’s abilities and need for assistance and continued therapeutic intervention.

4. Recommends modification or changes in the patient's treatment program to the referring physician based on evaluation of progress. Confers as necessary with physicians concerning alternatives available.

5. Prepares written/computer documentation as required by the profession and the department, such as evaluation results, treatment plans, progress reports, and other such reports.

6. Participates in the interdisciplinary patient care conferences and rounds to ensure efficient coordination of the overall health care plan. Communicates with other professional staff and patient's family/support system to provide continuity of care.

7. Arranges work schedule to make efficient use of direct and non-direct patient care time. Provides weekend coverage according to guidelines of rotation/department.

Requirements

Education and/or Experience
  • A Bachelor’s degree is required, Master’s and/or Doctorate Degree in Physical Therapy is preferred
  • Must have minimum 1 year of experience
  • Current Illinois license in Physical Therapy
