The Association for Individual Development (AID) is a non-profit organization whose mission is to empower people with physical, developmental, intellectual, mental health challenges, those who have suffered a trauma, and those at risk to enjoy lives of dignity and purpose. We are looking for a Case Manager Mental Health Professional Bilingual HOPWA, Elgin, IL who exemplifies that mission, and wants to work for an organization that makes a difference.

Schedule: M-F 8-4:30

What will you be doing? The CM MHP is responsible for managing individuals’ housing including: all paperwork needed to remain in compliance with TCHIP/HOPWA program requirements/rules, assisting individuals in finding and obtaining appropriate housing, maintaining leases, developing and maintaining good working relationships with landlords, providing medical case management and behavioral health services as appropriate.

The CM MHP is expected to obtain and maintain all needed trainings to remain in compliance with TCHIP/HOPWA requirements. The CM MHP is expected to participate in ongoing meetings and any other required committees needed to facilitate good working relationships with the HOPWA team as a whole including surrounding area HOPWA teams.

The CM MHP must complete documentation of services including mental health services and HOPWA related services provided in accordance with state and agency standards as applicable.

The CM MHP is expected to work with a team of behavioral health service providers within the agency as well as outside agencies, if applicable, to ensure positive client-centered approach.

Essential Job Responsibilities

  • Work scheduled hours and be flexible to meet client and program needs, as assigned by Program Manager or Director.
  • Recognize emergency situations and take appropriate action. Contact Manager and Director per procedure. Complete necessary paperwork correctly (incident reports, petitions, case notes).
  • Assure compliance with all agency, state and federal regulations while providing services and completing assignments. Review and follow updated policies and procedures.
  • Facilitate communications and coordination of services with other AID staff and professionals in the community.
  • Maintain professionalism and good boundaries when working with clients, coworkers and outside agencies.
  • Meet minimum service hour provision productivity standards (MRO) monthly.
  • Complete all required case management documentation (assessments, IM+CANS, consents, TCHIP/HOPWA paperwork, residential paperwork) on a timely basis.
  • Complete and sign all MRO Documentation within 48 hours using Cx360
  • Complete Case Worthy documentation by the 2nd of each month for the previous month.
  • Meet with all assigned clients on regular basis depending on program and client needs; turn in completed daily activity logs.
  • Develop, review and revise the IM+CANS and complete corrections within the timeframe allotted.
  • Complete assessment tools and necessary paperwork (including Locus, SAMA, consents, client rights, and privacy practices) per program procedure.
  • Assist clients with activities of daily living, including apartment maintenance, and provide client training consistent with the treatment goals.
  • Provide guidance to MHP and RSA staff regarding implementation of IM+CANS goals/objectives.
  • Obtain Input from clients, families, guardians and other staff on how to improve residential services.
  • Monitor and report safety of clients’ living arrangement
  • Acquire and maintain required trainings and certifications as well as any other trainings assigned by Manager.
  • Assist clients in paying bills, budgeting and banking per program procedure.
  • Assist client with completing TCHIP, IDPH, rent renewal, and other housing paperwork in a timely manner to help maintain clients housing
  • Oversight or management of client funds when necessary (dual signature accounts, AID rep. Payees).
  • Obtain and maintain client benefits (Social Security, Medicaid, Link Card, Etc).
  • Assure client records are properly maintained per agency procedures including scanning necessary paperwork into Cx360 per procedure.
  • Complete authorizations, reauthorizations and spend-down paperwork in a timely fashion.
