As a Home Health PT at Inspired Homecare, you'll have an exciting and fulfilling role that allows you to directly impact the lives of our patients. Your day-to-day responsibilities will involve conducting comprehensive assessments, creating personalized treatment plans, and delivering high-quality care to individuals in the comfort of their homes. In this dynamic position, you can expect to work closely with a diverse team of healthcare professionals to ensure seamless coordination of care.

You'll be responsible for monitoring patient progress, adjusting treatment plans as needed, and providing ongoing support to help patients achieve their goals.

Here’s what we’re looking for:

1. A degree in Physical Therapy from an accredited university/program.

2. A current, unrestricted state license as a Physical Therapist in the state of practice.

3. Demonstrated proficiency in clinical assessments, documentation, and compliance with policies and procedures.

4. Strong organizational and communication skills.

5. Experience in Home Health Therapy and completion of OASIS documentation is preferred.
