Job DescriptionThe After Hours RN is a professional caregiver who is a member of the interdisciplinary team. Responsibilities include the coordination and provision of direct and indirect patient care using the nursing process to meet the physical, psychosocial, environmental, and spiritual needs of Carle Home Services specific patient populations and families throughout the geographical area. The after‑hours nurse provides for continuity of care by coordination of care through triaging, patient visits and admissions.
This position will be responsible for the services that are provided when the hospice office is closed through phone triage, patient visits and admissions.
- Licensed Registered Professional Nurse (RN) – Illinois Department of Financial and Professional Regulation (IDFPR)
- Proof of Auto Insurance – varies
- Basic Life Support (BLS) within 30 days – American Heart Association (AHA)
- Driver’s License – Illinois Secretary of State (ILSOS)
- Education:
Associate’s Degree - Work Experience:
1 year of nursing experience
- Provides on‑call coverage and performs patient visits as assigned and needed for appropriate hospice care.
- Provides visits and consultation to patients, families, and health care providers regarding hospice services and end‑of‑life treatment choices and performs the admission process.
- Maintains clinical records in accordance with policies, procedures, state and federal regulations, and accreditation standards.
- Coordinates all patient care needs during evenings, nights, holidays, and weekend hours.
- Maintains up‑to‑date patient records with accurate documentation of problems, plans, actions, and goals.
- Provides patient/family visits based on assessed need, hospice policy, procedure, and established schedule.
- Follows established protocols for symptom management and patient visits.
- Communicates scope of hospice care to patients, families, and health care providers as needed.
- Assesses referrals for medical, psychosocial, and environmental appropriateness for hospice care in conjunction with the attending physician and interdisciplinary team.
- Contacts attending physician to obtain verbal certification of prognosis and medical information.
- Determines if patients are appropriate for hospice care using established criteria.
- Collaborates with attending physician to confirm existing orders, change orders, and acquire new orders.
- Obtains informed consents for hospice care when needed.
- Completes hospice admissions in home, nursing home, and hospital settings.
- Coordinates delivery of appropriate medications, durable medical equipment, and supplies.
- Records all patient/family visits, telephone contacts, and referral actions in the medical record.
