Palliative Nurse Practitioner in Lisle, IL

Job Description

We are looking for compassionate nurse practitioners with:

• Bachelor of Science (BSN) degree from an accredited School of Nursing
• Master of Science (MSN) degree from an accredited School of Nursing
• A minimum of five (5) years of clinical experience
• Hold, in good standing, a license as a Registered Nurse and Nurse Practitioner issued by the state in which you work
• Experience in a community based Home Health or Hospice/Palliative Care practice is strongly preferred.

• Provide management and delivery of patient care plans in facilities and communities
• Collaborate with the care team
• Support patients in Lisle, IL and surrounding areas

• Mon-Fri; 8A to 5P. No weekends/holidays/on calls
