Home Care Director oversees daily operations of branch location(s), ensures quality patient care, maintains regulatory compliance, and leads staff.
Responsibilities- Oversee daily operations of branch location(s), including staffing and scheduling.
- Ensure quality patient care standards are met and that regulatory agencies' requirements are satisfied.
- Provide leadership, coaching, and performance evaluation to staff, fostering a culture of service excellence.
- Conduct cascade learning sessions, submit accountability forms, and participate in daily charge meetings.
- Use effective communication techniques (e.g., SBAR) and active listening when interacting with staff, physicians, and patients.
- Develop staffing schedules to maximize utilization of human and material resources.
- Resolve conflicts and develop win-wins solutions for complex problems.
- Promote and apply Quality Improvement principles to continuously improve departmental services.
- Engage in lifelong learning by participating in professional associations and pursuing continuing education.
- Maintain accurate documentation that meets legal and regulatory standards, supports clinical continuity, and ensures proper medical necessity for reimbursement.
- Coordinate patient intake, care plan development, and resource utilization, ensuring proper matching of caregiver qualifications.
- Lead interdisciplinary case conferences to address patient needs and create individualized care plans.
- Develop, implement, and evaluate departmental policies, procedures, protocols, and standards.
- Advocate for a safe environment, encouraging reporting of safety events and implementing improvement plans.
- Represent the Home Care program in the community, promoting services to physicians and the public.
- Commit to service excellence, monitor metrics, and lead service recovery initiatives.
- Manage staff time, attendance, and performance reviews, ensuring budget and operational objectives are met.
- Model integrity and ethical standards, using employee survey results to improve performance.
- BSN (required).
- Registered Nurse (RN) with an active license in Illinois.
- Experience as a nurse with a strong commitment to quality patient care.
Estimated range: $99,112.00 – $, payable based on experience. Eligible for Management Incentive Compensation Program.
