Nurse Practitioner No Nights or Weekends

A specialized infectious disease practice serving skilled nursing and long-term care facilities across the Chicagoland area is seeking an experienced Nurse Practitioner (FNP) for a part-time multi-site role in Macomb, IL.

This is a focused, mission-driven clinical position — diagnosing and managing infectious diseases in a geriatric long-term care population, reducing rehospitalizations, and serving as a key clinical leader across assigned facilities — all on a clean Monday–Friday schedule with no nights, no weekends, and flexible daily start times.

• Diagnose, treat, and manage infectious diseases across skilled nursing and long-term care facilities
• Perform comprehensive physical assessments and monitor changes in patient conditions
• Prescribe medications and develop individualized treatment plans
• Participate in and guide residents' plans of care
• Coordinate advanced care planning and discharge processes
• Facilitate referrals and collaborate with physicians and specialists
• Maintain a minimum of 12 patient visits per day (bonus eligible for exceeding targets)
• Ensure timely, accurate documentation and communication across facilities

• MSN from accredited NP program required
• 4+ years NP experience; infectious disease experience strongly preferred
• Experience in acute, sub-acute, or long-term care settings required
• Background in geriatrics, internal medicine, primary care, hospice, or palliative care preferred
• Knowledge of Medicare, Medicaid, and managed care structures a plus
• Strong interpersonal skills with elderly populations

• $50–$60/hour + monthly performance bonus
