We are hiring a Certified Nursing Assistant for a Hospice position providing care to patients in their homes in the Elgin, IL area and are eligible for a $5,000 Sign-on Bonus.
What You’ll Do:- Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life.
- Provide hands‑on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life.
- Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey.
- Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR.
- Work independently, but never alone.
- Travel to patient homes within assigned territory.

- Pathway to Nursing Program:
