Assists RN with daily care of patients including bathing, oral hygiene, feeding, activity orders and ambulation; measures and documents vital signs and intake/output; documents in EMR; participates in admission/transfer/discharge processes under supervision; conducts purposeful rounding and communicates patient needs to RN; assists with positioning, transporting, and use of assistive devices; supports nutritional and skin-care tasks; collects specimens and may perform limited procedures;
maintains infection prevention practices; coordinates tests and treatments and manages communications; maintains documentation and patient records; fosters positive patient relations and age-appropriate care.
