Position Summary:
As a Psychiatric Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will provide behavioral health and psychiatric medication management services within skilled nursing and senior living communities. This role includes routine patient rounding, evaluation and ongoing management of mental health conditions, and close collaboration with interdisciplinary teams to support residents' emotional and cognitive well-being. You will work closely with facility partners to ensure continuity of care, optimize treatment plans, and promote a therapeutic environment.

Qualifications:
- Current NP or PA licensure in the state of practice
- Previous professional patient care experience
- Experience in geriatrics, family medicine, or psychiatry preferred.
- Exceptional patient-centered service skills, including compassionate communication, active listening, professionalism, and the ability to build trusting relationships with residents, families, and facility staff
- Strong collaboration skills with the ability to work effectively across interdisciplinary teams
