Gateway Community Health Centers is looking for a passionate and dedicated Family Nurse Practitioner to join our team in Gatesville, NC. Responsibilities:- Provide comprehensive primary care services to patients across the lifespan, including pediatric, adult, and geriatric populations
- Conduct thorough patient assessments, physical exams, and develop individualized care plans
- Diagnose and manage acute and chronic conditions including diabetes, hypertension, and respiratory illness
- Order and interpret diagnostic tests, labs, and imaging to guide clinical decision-making
- Prescribe medications and therapies in accordance with evidence-based guidelines and state regulations
- Provide patient education on disease prevention, health promotion, and chronic disease management
- Collaborate with physicians, specialists, and care team members to ensure coordinated, whole-person care
- Maintain accurate and timely documentation in the Electronic Health Record (EHR) system
- Current Family Nurse Practitioner (FNP) certification from AANP or ANCC
- Active or eligible Nurse Practitioner licensure in the state of North Carolina
- Master's or Doctoral degree in Nursing (MSN or DNP) from an accredited program
- DEA registration and prescriptive authority (or ability to obtain)
- Experience in primary care, community health, or federally qualified health center (FQHC) settings preferred
- Proficiency with Electronic Health Records (EHR); experience with eClinical
Works a plus - Strong interpersonal and communication skills with a patient-centered approach
- Commitment to serving underserved and diverse patient populations
