hospice registered nurses needed in ocean county, nj. This is a hospice home health care role. This is a travel role that is a 6 month contract role that can extend after the six months. Must have hchb (homecare homebase) documentation experience for this opening. Salary range for this role is 85-100k. Must have be a rn and have a valid driver's license and vehicle.
purpose: the registered nurse, rn, is a member of the interdisciplinary team (idt) and is pivotal in identifying the physical, psychological, social and spiritual needs of the patient and family. The rn initiates the appropriate interventions and support of the patient and family upon admission provides a continuous, appropriate, comprehensive and responsive plan of care.
qualifications:
nj registered nurse license in good standing required
minimum two (2) years nursing experience in an acute-care hospital nursing in either medical, surgical or oncology unit or other commensurate experience
current clinical skills and a strong customer service focus
knowledgeable in caring for the terminally ill patient
communication skills (compassionate nature, excellent interpersonal skills, strong verbal and written skills)
responsibilities (including, but not limited to):
this position does have supervisory duties.
assess patient needs in the home or facility, being aware of the physical, emotional, and spiritual aspects and gather data on social, economic and cultural factors which may influence health, well-being and quality of life.
provide direct patient care to patients as determined by the individualized plan of care in order to maintain the highest level of comfort and quality of life, assuming primary responsibility for case management.
coordinate patient/family plan of care and services including the interdisciplinary team. Evaluate and perform ongoing assessment, develop and revise as needed initial plan of care with collaboration from team physician and idt members.
educate patient/family and other primary caregivers in appropriate care techniques.
serve as the patient/family advocate by maintaining regular communication with attending physicians, assisted living, long term care facility staff, hmo case managers and others.
appropriately utilize the skills and resources of contract personnel when appropriate i.e., physical therapy.
assess the physical and psychosocial status of the patient/family within 48 hours of admission. Update in brightree within 48 hours of completing the assessment.
may participate in providing bereavement support to families as well as associated bereavement activities.
ensure and document appropriate use of medications, durable medical equipment and supplies, e3 mobile app (enclara) for medication profiling.
participate in team meeting. Attend and participate in interdisciplinary team meeting (idt), reporting necessary, information to the regional manager and all members of the idt.
provide accurate documentation with visit itineraries to the office. Complete documentation as outlined in hospice, state and federal guidelines/standards. Documentation must be prepared and turned in according to these guideline
