We are seeking compassionate and driven Nurse Practitioners to join our dynamic Clover Home Care teams in Gloucester County, NJ. This is a unique opportunity to provide comprehensive primary and supportive care directly in the homes of vulnerable members, improving health outcomes and quality of life.
Job OverviewAn NJ registered Nurse Practitioner with advanced practice certification will join the Clover Care Visit (CCV) Team, focusing on early intervention and transitional care through in‑home visits, leveraging technology to support decision‑making and improve outcomes.
Responsibilities- Conduct comprehensive in‑home health assessments for enrolled Medicare Advantage members.
- Perform post‑discharge follow‑up visits to reduce hospital readmissions.
- Collaborate with interdisciplinary teams using data‑driven clinical tools.
- Located in or near Gloucester County, New Jersey and able to travel locally for home visits.
- Active, unencumbered NJ Registered Nurse license.
- Certified Advanced Practice Nurse Practitioner (APN).
- Master’s degree or post‑master’s certificate in advanced clinical practice.
- Minimum of 1 year of NP experience in a clinical setting.
- Active DEA and CDS registration.
- Current Basic Life Support (BLS) certification.
- Experience in home care or house call settings (preferred, but not required).

Base salary range: $140,000 – $165,000 USD.
