Responsibilities
- Conduct in-home assessments using comprehensive tools
- Create and manage individualized care plans
- Coordinate services such as adult/pediatric daycare, personal care, and nursing facility placements
- Collaborate with members, families, providers and interdisciplinary teams
- Ensure cost‑effective service delivery and accurate documentation
- Mentor new hires once proficient
- Travel 50–75% within assigned counties
- Active and unrestricted RN license in NJ required
- Must reside in Somerset County, NJ
- 3+ years of clinical practice experience (e.g., hospital setting, home health, or ambulatory care) required
- Successful completion of the NJ Choice certification is a condition of employment
- As required by the NJ Division of Aging Services, Office of Community Choice Options guidance, an exam score of 80% or higher is required
- Must possess reliable transportation and be willing and able to travel up to 75% of the time
