LPN Supervisor

Aboutthe Role
- Supervise and mentor LPNs and other nursing staff to deliver exceptional patient care
- Develop and implement effective care plans, ensuring patient-centered care and satisfaction
- Conduct daily observations and assessments, reporting any changes or concerns to the RN Supervisor
- Collaborate with interdisciplinary teams to provide comprehensive care services
- Maintain accurate and up-to-date patient records, investigating and resolving any discrepancies
- Participate in performance improvement initiatives, identifying opportunities for growth and improvement
- Supervise and coordinate the work of LPNs and other nursing staff
- Ensure compliance with regulatory requirements, such as CMS guidelines and state nurse practice act
- Develop and maintain strong relationships with patients, families, and healthcare teams
- Coordinate with physicians and other healthcare professionals to ensure seamless care transitions
- Monitor and report any changes in patient condition or status to the RN Supervisor
- Participate in quality improvement initiatives, identifying and implementing best practices

- 2+ years of experience as an LPN (with a valid NJ LPN license)
- Strong leadership and communication skills
