Job Summary:

At Elizabeth Park Rehabilitation and Nursing LLC, a leading provider of residential, long-term, rehabilitation, and subacute care, we are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team. As a LPN, you will play a vital role in providing high-quality care to our patients and residents. Responsibilities:
  • Provide direct care to patients and residents, including administering medications, wound care, and other treatments as prescribed by physicians

  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams

  • Monitor and report patient progress, identifying areas for improvement and making recommendations for modifications to care plans

  • Collaborate with other healthcare professionals, including RNs, PTs, OTs, and social workers, to ensure comprehensive care

  • Participate in patient and family education, promoting effective communication and empowerment

  • Maintain accurate and detailed records, adhering to regulatory and facility standards

Requirements:
  • Current LPN licensure in the state of New Jersey

  • High school diploma or equivalent required; ADN or BSN degree preferred

  • 1+ year of experience as an LPN in a long-term care or rehabilitation setting; experience in rehabilitation and nursing highly valued

  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams

  • Ability to lift, transfer, and reposition patients, as well as perform other physical demands of the role
