Licensed Practical Nurse

Eagle Rock Post Acute Care at Cedar Grove seeks a compassionate Licensed Practical Nurse to provide bedside care, medication administration, and clinical support for post-acute and long-term residents in our skilled nursing facility. The LPN will perform assessments, monitor vitals, support wound care and treatments, and document in the EHR while collaborating with RNs, therapists, and physicians. You'll educate residents and families, uphold infection control and safety standards, and contribute to person-centered care plans in a respectful, team-focused environment.
