About This Role

The RN Field Assessor role at CVS Health involves conducting face-to-face assessments to evaluate members' medical needs and overall wellness, providing an opportunity to impact lives through holistic, person-centered care.

  Trenton, NJ
• Residency:
  Must reside in Passaic, Morris, Sussex or Warren Counties, NJ
• Travel:
  Up to 85% of the time
• Schedule:
  
  Full-time, Monday–Friday, 8:00 AM–5:00 PM EST

• Conduct face-to-face assessments with members to evaluate medical needs and overall wellness.
• Help members access vital services such as Adult & Pediatric Medical Daycare and Personal Care Assistance.
• Complete 20+ assessments per week using a holistic approach.
• Collaborate with your team and supervisor to overcome barriers and achieve goals.
• Use interviewing skills to engage members and uncover their health needs.
• Mentor new team members once you’ve mastered your role.
• Operate at full capacity by month four with comprehensive training and support.

Education:

• Associate Degree in Nursing required; BSN preferred.

Experience:

• 1 or more years clinical experience (hospital, home health, or ambulatory care).

Licenses &

Certifications:

• Active, unrestricted RN license in New Jersey required.
• Successful completion of the NJ Choice Certification, including minimum exam score of 80%.
