Care Manager Registered Nurse - Field- Essex

About This Role

The Care Manager Registered Nurse position at CVS Health involves assessing, planning, implementing, and coordinating case management activities for members, particularly those enrolled in Medicare and Medicaid. • Residency:
  Must reside in Essex County, New Jersey or within surrounding counties within 50 miles of Essex.
• Schedule:
  
  Full-time, Monday – Friday, 8-5 pm EST

• Use clinical tools and information/data review to evaluate member's needs and benefits.
• Apply clinical judgment to reduce risk factors and address complex health and social indicators.
• Conduct assessments considering information from various sources, including claims.
• Use a holistic approach to assess the need for referrals to clinical resources and interdisciplinary team members.
• Collaborate with supervisors and key stakeholders to overcome barriers in meeting goals.
• Present cases at interdisciplinary case conferences.
• Utilize case management processes in compliance with regulatory and company policies.
• Employ motivational interviewing skills to engage members and assess their health needs.

Education:

• Associate's Degree required

Experience:

• Minimum 3+ years of clinical practice experience
• Minimum 2+ years in Care Management, Discharge Planning, and/or Home Health Care Coordination preferred

Licenses &

Certifications:

• Active and unrestricted RN licensure in the state of NJ required
• Certified Case Manager preferred
