About the Role:

As an LPN Supervisor at Elizabeth Park Rehabilitation and Nursing, you will play a vital role in supervising our nursing team and ensuring the highest standards of patient care. Your comprehensive nursing experience and leadership skills will enable you to:

  • Supervise and mentor LPNs and other nursing staff to deliver exceptional patient care

  • Develop and implement effective care plans, ensuring patient-centered care and satisfaction

  • Conduct daily observations and assessments, reporting any changes or concerns to the RN Supervisor

  • Collaborate with interdisciplinary teams to provide comprehensive care services

  • Maintain accurate and up-to-date patient records, investigating and resolving any discrepancies

  • Participate in performance improvement initiatives, identifying opportunities for growth and improvement

Responsibilities:
  • Supervise and coordinate the work of LPNs and other nursing staff

  • Ensure compliance with regulatory requirements, such as CMS guidelines and state nurse practice act

  • Develop and maintain strong relationships with patients, families, and healthcare teams

  • Coordinate with physicians and other healthcare professionals to ensure seamless care transitions

  • Monitor and report any changes in patient condition or status to the RN Supervisor

  • Participate in quality improvement initiatives, identifying and implementing best practices

Requirements:
  • 2+ years of experience as an LPN (with a valid NJ LPN license)
