About the Role

The Clinical Account Liaison (CAL) is Senior Care Therapy’s embedded clinical liaison within an assigned portfolio of approximately 20 skilled nursing facility (SNF) partners. The CAL ensures that residents who would benefit from psychiatric and psychological services are identified, appropriately referred, and connected to SCT’s clinical team in a timely, medically necessary manner.

Working side-by-side with Directors of Nursing (DON), Social Services Directors, MDS Coordinators, consultant pharmacists, unit managers, and other members of the interdisciplinary team (IDT), the CAL strengthens the clinical fabric of each facility partnership, supports appropriate psychotropic medication management, and serves as a continuous education and clinical resource on SCT services. The CAL deepens and stewards SCT’s existing facility partnerships through clinical credibility, education, and embedded relationship work.

What You’ll Do:
  • Serve as the embedded, recognizable clinical point of contact for an assigned portfolio of approximately 20 SNF partners.
  • Build and maintain trusted, peer-level working relationships with DONs, Administrators, Social Services Directors, MDS Coordinators, consultant pharmacists, ADONs, unit managers, and other IDT members.
  • Function as a clinical resource and educator on SCT’s services, clinical scope, and referral pathways for facility staff at every level.
Clinical Identification & Medically Necessary Referrals
  • Partner with facility clinical staff to identify residents with unmet psychiatric and psychological needs.
  • Facilitate medically appropriate referrals into SCT services for new admissions and existing residents with changes in condition, behavior, mood, cognition, or medication regimen.
  • Collaborate with consultant pharmacists and prescribers to support appropriate psychotropic medication management, including gradual dose reductions (GDRs) and antipsychotic use aligned with CMS guidance.
  • Help facility teams recognize clinical trigger events that warrant psychiatric or psychological evaluation — behavior incidents, new psychotropic orders, schizophrenia diagnostic validation, depression and anxiety presentations, and changes in cognition or function.
  • Conduct ongoing in-services and education for nursing, social services, and clinical leadership on SCT’s services, clinical scope, and referral process.
  • Place, maintain, and regularly refresh SCT reference binders at every nursing workstation in each assigned facility — including current contact cards, on‑call numbers, referral pathways, and concise service summaries.
  • Provide just‑in‑time coaching to nursing and social services staff on identifying behavioral health needs and initiating appropriate referrals.
