Case Manager Registered Nurse - Field in Hudson & Counties

About This Role

The Case Manager Registered Nurse position is a field-based role that involves significant travel to provide care management services in Hudson and surrounding counties. This role is ideal for experienced nurses who are skilled in clinical judgment and critical thinking, and who are passionate about delivering quality healthcare to members.

• Pay: $66,575 - $142,576/year
• Location: New Jersey, NJ
• Schedule: Full-time, Monday - Friday, 8am-5pm EST
• Travel: Up to 75% travel required in Hudson and surrounding counties

• Conduct face-to-face visits in members' homes using comprehensive assessment tools.
• Evaluate medical needs of members enrolled in Managed Long-Term Services and Supports (MLTSS) and Dual Special Needs Program (D-SNP/FIDE).
• Facilitate members' overall wellness by addressing requests for various services.
• Schedule and attend interdisciplinary meetings advocating for members' safe discharge.
• Develop care plans and authorize services within the MLTSS/FIDE benefit.
• Document accurately and timely in the member's electronic health record.
• Coordinate and collaborate care with members, PCPs, and care team participants.
• Mentor new hires once proficiency is attained in the role.

• Minimum associate's degree or diploma in Nursing required

• 3+ years clinical practice experience in a hospital or alternative care setting

• Active and Unrestricted RN license in NJ
• Successful completion of the NJ Choice certification is a contingency of employment
