Registered Nurse Case Manager – Field; Monmouth & Counties, NJ

About This Role

The Registered Nurse Case Manager – Field role at CVS Health involves coordinating care for members with complex needs, primarily in a field-based setting. This position is ideal for compassionate and experienced nurses who enjoy working independently and engaging with the community.

  Full-time, Monday–Friday, 8:00 AM – 5:00 PM
• Travel:
  Up to 75% travel required within the county of residence and surrounding areas

• Conduct in-home assessments with members enrolled in Managed Long-Term Services and Supports (MLTSS) programs
• Evaluate medical, behavioral, and social needs to support appropriate care and services
• Collaborate with providers, caregivers, and interdisciplinary teams to ensure coordinated, holistic care
• Develop and manage individualized care plans that promote safety, independence, and quality of life
• Participate in care team meetings and advocate for member needs
• Document all assessments and care activities accurately in the electronic health record
• Support members through care transitions and help connect them with community resources
• Mentor new team members as you grow in the role

Education:

• Associate’s degree in Nursing required, BSN preferred

Experience:

• At least 3 years of clinical experience (hospital, home health, ambulatory care, or similar settings)

Licenses &

Certifications:

• Active, unrestricted RN license in New Jersey required
• Successful completion of the NJ Choice Certification, including minimum exam score of 80%, completion of state training modules, and field mentoring component
