Position Summary
The Behavioral Health Clinical Case Manager conducts comprehensive in‑person assessments and develops individualized care plans for members in MLTSS and D‑SNP programs. The role coordinates physical, behavioral, and social services across providers and community resources, ensuring members receive appropriate, high‑quality, and cost‑effective care. The Case Manager authorizes services, documents care activities, participates in interdisciplinary meetings, and supports team development. Flexible teleworking and meaningful community engagement provide opportunities to build trusted relationships and advocate for vulnerable populations.
Responsibilities- Conduct comprehensive, face‑to‑face assessments for members enrolled in MLTSS and D‑SNP programs.
- Develop and implement personalized care plans that address physical, behavioral, and psychosocial needs.
- Collaborate closely with members, families, providers, and interdisciplinary teams to coordinate care.
- Evaluate and facilitate services such as adult and pediatric medical daycare, personal care assistance, nursing facility support, and community‑based long‑term services.
- Advocate for members to ensure access to appropriate and high‑quality care.
- Authorize services and ensure efficient, cost‑effective resource utilization.
- Document care activities accurately in the electronic health record.
- Participate in interdisciplinary meetings and help drive positive outcomes.
- Support and mentor new team members as you grow in your role.
- Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW).
- At least 2 years of Behavioral Health experience/expertise.
- Reliable transportation and willingness to travel up to 75% of the time; mileage reimbursed per company policy.
- Successful completion of the NJ Choice certification (score of 80% or higher on the exam, completion of state training modules, and field mentoring component).
- Master’s Degree and independent license in NJ LSW or LCSW.
- Experience in case management or discharge planning.
- Managed care experience.
