Responsibilities
  • Coordinate admissions of hospice patients by making skilled assessments of patients' initial physical, psychosocial and spiritual needs.
  • Coordinate the establishment of the initial plan of care with the attending physician, medical director and the interdisciplinary team.
  • Provide skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of the physician.
  • Function as a liaison between disciplines and facilities during the admission process.
  • Develop, prepare and maintain individualized patient care progress records with accuracy, timeliness and according to policies. Submit accurate documentation per care center policy. Record pain/symptom management changes/outcomes as appropriate.
  • Communicate significant findings, problems, changes in condition or environment to the clinical manager/physician and other members of the hospice care team. Communicate with the physician regarding the patients' needs and report changes in the patient's condition; obtain physician orders as required.
  • Perform on-call responsibilities and on-call services to patients/families as assigned.
  • Perform other duties as assigned.
Qualifications
  • Current RN license, specific to the state(s) you are assigned to work.
  • One year of clinical RN experience.
  • Current CPR certification.
