• 1:1 social worker‑to‑patient ratios where you impact the quality of life outcomes
• Flexible assignments, autonomy, and work‑life balance
• Online training, growth, and ability to earn CEUs

• Provide home‑based social services to patients dealing with end‑of‑life issues including care management, pain, symptoms associated with chronic and terminal illnesses, depression, anxiety, poor nutrition, or a lack of resources that make managing their journey difficult
• Work as part of a hospice team which will include an RN, LPN, CNA, Spiritual Counselor, Volunteers, Hospice Aides, PT, OT, or SLP, focused on the patient’s plan of care and individual goals
• Monitor a patient’s psychosocial condition and identify social and emotional needs
• Conduct patient assessments, document progress, and ensure patient is moving toward goals
• Provide counseling, community resource planning, crisis intervention, and advocacy
• Consult with family and caregivers on the patient’s plan of care and how to help them progress

• Master’s degree in Social Work and active MSW license in Ohio
• Minimum of 2 years of social work experience, ideally in home healthcare
• CPR certification - Can provide if needed
• Strong interpersonal skills, good communicator, empathetic, compassionate, and resourceful
